Private health insurance
Not sure if you have private health insurance coverage?
You are not alone. Many patients have insurance plans with unclear alternative healthcare benefits. Because it is so important to understand your out-of-pocket responsibilities before you receive care, we will contact your insurance company before your appointment to get a rundown of your benefits.
We appreciate appointments that are made at least 48 hours in advance to allow our insurance specialists time to connect with your plan.
In-Network Health Insurance Plans:

- PacificSource
- Providence Health Plans

Out-of-Network Insurance Plans:
While we are out-of-network with all other health insurances, there are many insurers we work with who offer out-of-network chiropractic benefits, such as United Healthcare. We are happy to contact your insurance company to see what is covered!

- Medicaid
- Oregon Health Plan (OHP)
- Cigna
- International Longshore and Warehouse Union (ILWU) plan through First Choice Health Network
**If your primary insurance is Medicare, please call our office to discuss what is covered. Or read more about Medicare in our FAQ.
Please note: We work hard to verify your coverage before your appointment. However, if we are unable to verify your insurance benefits by the time you come in for treatment, you will be charged a placeholder fee of $250.00 at the time of your appointment. This fee will be refunded if your insurance subsequently covers your visit.
Auto accident and Personal Injury Protection (PIP) insurance
Were you injured in a car accident? Oregon is a "no-fault" state when it comes to car accidents, and all auto insurance policies carry personal injury protection (PIP) coverage. This means that regardless of who was at fault, your medical treatment will be covered by your auto policy's PIP coverage, usually at no cost to you! Learn more about what to do after an auto injury.
- The first step is to file a claim with your auto insurance company.
- The second step is to schedule an appointment at Hosmer Chiropractic Health for some much needed treatment!
- Scheduling is easy! We will need the name of your insurance company, your claim number, the name and contact information of your adjustor, and we'll do the rest.
Please note: We do not bill third party claims that are pending settlement to pay for your medical treatment.
On-the-job injury/workers compensation
Our chiropractors would love to help you recover from your workplace injury! However, worker's compensation regulations in the State of Oregon are complicated and your treatment may not be covered at our clinic. Call us with the details of your injury, and we will help find out if your treatment will be covered at Hosmer Chiropractic Health.
No insurance or alternative healthcare coverage
No insurance? No problem! We participate in a discounted medical program through ChiroHealth USA. Similar to a Costco membership, ChiroHealth USA members are eligible for discounted rates for treatment at our clinic! Enrollment will be available at the time of your first visit, and payment for enrollment and for treatment are due at the time of service.
The costs for enrollment and treatment through ChiroHealth USA are as follows:
- ChiroHealth USA annual enrollment fee (includes dependents): $49.00
- New patient visit ($250.00) and re-exam visits ($175.00)
- Follow-up chiropractic visit (treatment): $125.00
- Massage (60 minutes): $115.00
